A person who is responsible for providing nursing care services according to the bio-psycho-social needs of users or groups under her/his care. Under the authority of the Nursing Coordinator, the nurse provides services to clients of all ages, including a large proportion of pediatric clients. The Chisasibi Regional Hospital has 29 beds for acute care (5 pediatric), chronic care and respiratory care. In addition to the permanent general practitioners in Chisasibi, various specialists visit the territory on a regular basis to provide services in obstetrics, surgery, pediatrics, orthopedics, internal medicine, ophthalmology, otorhinolaryngology and psychiatry. The National Native Alcohol & Drug Addiction Program (NNADAP) worker develops and runs prevention, intervention and aftercare services and activities that aim to reduce or prevent alcohol and drug addiction in Eeyou Istchee. More specifically:
• Helps individuals and families that may be struggling with substance use by establishing and running education, awareness and prevention initiatives;
• Collaborates with regional and local partners to reduce substance abuse and dependencies, and thereby contribute to the well-being of Cree individuals, families, and communities;
• Organizes, coordinates and implements a range of prevention, intervention and aftercare activities that are suited for the individual such as: workshops, recreation activities, discussion groups, public awareness campaigns, spiritual and cultural programs’ activities and other community activities related to drug and alcohol abuse;
• Participates in school programs and cultural events;
• Participates actively in aftercare activities that includes crisis intervention, counselling services, sharing circles and support groups, support visits and referrals to other services, including detox and medical treatments, social services, and band services;

DESCRIPTION
Person responsible for nursing care and professional medical activities for users in a specialty area dealing with complex health problems, in accordance with current legislation. Collaborates with the attending physician to identify health problems and establish care and treatment priorities. A licensed health professional authorized to practice autonomously within their clinical specialty, the Specialized Nurse Practitioner in Mental Health (NP-MH) ensures the assessment, diagnosis, treatment, and follow-up of individuals with mental health issues in accordance with the applicable legislative and regulatory framework. Under the authority of the Clinical Nursing Advisor and the Director of Nursing, they will contribute to the accessibility and continuity of mental health care in close collaboration with psychiatrists, mental health nurses, and various psychosocial workers in the pole. The role includes a hospital component - notably the initial clinical assessment of patients admitted in psychiatric emergency at the Chisasibi hospital - as well as a community component that will develop gradually (consultations, follow-ups, clinical monitoring and lab/examination follow-up in support of psychiatrists and teams). The position is based in the community of Chisasibi. Travel to communities within the pole will be required based on identified needs and the evolution of the service. The NP-MH must demonstrate resourcefulness, proactivity, and the ability to collaborate interdisciplinarily. They must also adapt their interventions to the cultural, social, and contextual realities of Cree communities, using a respectful, safe, and person-centred approach. • Conduct initial clinical assessments (mental status examination, risk appraisal, orientation) of patients admitted in psychiatric emergency at the Chisasibi hospital in collaboration with the psychiatry team;
• Provide advanced mental health nursing care: clinical monitoring, progress follow-up, complication prevention, and adjustments along the care trajectory (Assignment of a specific caseload is to be discussed);
• Participate in establishing the treatment plan (pharmacotherapy and non-pharmacological interventions) and in clinical follow-up, in collaboration with psychiatrists and the interdisciplinary team;
• Contribute to the follow-up of required tests (including laboratory work) and to the coordination of care, in accordance with established protocols and service pathways;
• Provide psychoeducation and support to clients and their families: understanding of the disorder, treatment, side effects, adherence, and self-management strategies;

The liaison nurse plays a key role in coordinating medical services for clients who require care from 2nd and 3rd line inside and/or outside of their community. In close collaboration with Wiichihiituwin and specialized services departments, the liaison nurse is responsible for organizing and preparing medical visits for clients from Eeyou Istchee who will meet specialists and their follow-up/care when they return in the community. This includes reviewing medical consultation requests and reports. Acting as a vital link between the client, hospitals, and Cree communities, the liaison nurse ensures that relevant medical information is shared with the appropriate healthcare professionals in the community, including the care team and extended team to ensure continuity of care. • Manage new consultation requests according to the defined service offer and service trajectories;
• Ensure validation of consultation forms and prerequisites before transferring request;
• Communicate with care team nurse to assure nursing care done before booking of the related consultation;
• Follow up on missing or expired documents (e.g., RAMQ) in collaboration with administrative staff;
• Use the digital platforms (e.g., Care4) to organize and manage the required documentation and track client records;
• Consider any delays, local medical follow-ups, and changes in the client’s medical condition that may occur while waiting for the specialist consultation, and communicate these to the specialist in collaboration with Wiichihiituwin and Specialized Services;
• Manage returns from medical visits, ensuring validation of medical reports and results;
• Notify the appropriate local care team nurse and other departments as needed of any new medical follow-ups or treatments required;

This includes services for conditions such as, but not limited to palliative care. Services are given where it is best suited in accordance with the clients’ needs.
