  • SUMMARY OF THE POSITION
    Person who is called upon to advise an institution on questions pertaining to nursing and who may be required to carry out such responsibilities as: The design, distribution, evaluation and revision of orientation, training and skills updating activities for employees and trainees; The design, implementation, evaluation and revision of programs aimed at improving the quality of health care and professional activities and programs. SPECIFIC FUNCTIONS Responsible for the planning of the training program of hemodialysis nurses and staff of the Cree Board of Health and Social Services of James Bay (CBHSSJB); Collaborate in planning the continuing education of nursing staff of the CBHSSJB in relation to the specialty; Collaborates in the development and updating of the content and teaching materials of the CBHSSJB nursing training program related to the specialty; Establish a schedule of training sessions and coordinates them; Participates in the orientation and welcoming of new nurses related to the specialty; Provides training sessions and assesses training outcomes for newly recruited nurses for duties related to the specialty; Provides preceptorship to nurses newly trained in connection with the specialty; May be called upon to give various training to existing nurses (annual nursing or PCCR training, training required by the MSSS, etc.); Participates in the process of observation and evaluation of the nursing staff in the optic of quality assurance of nursing activities; Collaborates in the development, revision, implementation and training related to clinical tools (therapeutic guide, collective prescriptions, Nephrocare, etc.); Work in collaboration with members of the CBHSSJB and MUHC nephrology team in the design, implementation, evaluation and revision of hemodialysis-specific services; May be called upon to advise the institution on matters related to nursing care or infection prevention and control programs; Participates in research and uses evidence-based data to identify strategies aimed at improving nursing professional development programs and activities; Contributes to the development of evidence-based nursing practice within their clinical area.     Education: Member in good standing of the O.I.I.Q; Bachelor of Science in Nursing or an undergraduate degree consisting of three (3) admissible certificates, of which two (2) are accredited certificates in Nursing. Certificate in Nephrology is an asset. Experience:

  • Person who participate in the identification of the needs of the population participate in the community organization & development and execute different action programs by using appropriate techniques, may act as a resource person for the groups. The incumbent participates in identifying the needs of the population served by the Kapataakan Waswanipi Pilot Project for the Community-Based Addiction/Wellness Recovery Program. He/she contributes to service delivery and project development, executes various tasks using appropriate techniques, and may act as a resource person for community partners. The mandate is to provide continuous first-line services to individuals, youth ages 12 to 17, and adults 18 and older experiencing problematic substance misuse, as well as to their significant relationships. Meet and intervene with individuals, families, and the community in accordance with the pilot project framework, while respecting CBHSSJB policies and procedures. • Assess the needs of those served, provide assistance, and refer them to appropriate programs within CMC resources or community partners when necessary. • Collaborate with therapeutic colleagues and clients to define, review, and update personalized care plans, ensuring aftercare plans are co-developed in line with client goals. • Ensure proper use of intervention and data collection tools. • Carry out prevention activities, such as presentations, in collaboration with community resources when requested. • Organize and facilitate information sessions and community activities. • Contribute to pilot project evaluation activities. • Perform any other duties related to the Community-Based Addiction/Wellness Recovery Program as requested by the supervisor.   • SUMMARY OF THE POSITION
    Person in a home, residence, group home or other facility of similar nature, who is responsible for a range of tasks aimed at supporting the user and his family or at compensating for his disabilities in performing daily activities or domestic tasks. Must also work at promoting the user's integration and socialization in individual and community activities.
    In the performance of his duties, the health and social services aide sees to users' hygiene, well being and comfort and meets their general needs. Installs certain devices and provides specific care for which he has been trained. Also prepares meals and performs domestic tasks.

    SPECIFIC FUNCTIONS
    The Following the policy and procedures of the C.B.H.S.S.J.B. and within the framework of the Youth Protection Act, Youth Criminal Justice Act, L.H.S.S.S. and C.M.C. mandate, the person will have to support, intervene, and refer the individuals, families & community to the appropriate resources in the organization. Assess service needs of youth/children and parents who are entrusted to him/her by virtue of the YPA or the Act respecting health & social services to support them and refer them as needed to the appropriate resources; More specifically, he/she will receive reports regarding children, analyze them briefly and decide whether they must be evaluated further; When required, he/she can refer the intervention for emergencies and support the evaluation and treatment in continuity in accordance to the laws in effect; According to the responsibilities for his/her job title, the position holder shall refer the individual or family to the Department of Youth Protection's regional services for the application of the psychosocial treatment plan required by the department's clientele.     SPECIFIC FUNCTIONS
    In accordance with the policy and procedures of the CBHSSJB and within the framework of the Youth Criminal Justice Act, Youth Protection Act, S-5. and CMC mandate, the incumbent: Carries out the provisions of the Youth Protection Act in accordance with the Reference Manual on the Youth Protection Act, the Cree Health and Social Services Act and all CBHSSJB's Policies and Procedures; Assesses, within required timeframes, the situation of children and families signaled to Youth Protection to permit the DYP to determine whether or not security or development is compromised in accordance with the factors of analysis contained in article 38.2 of the Youth Protection Act; Determines the degree of risk and, when appropriate, to determine the orientation; Intervenes on time in Application of Measures-oriented situations and implement Court-ordered measures or agreements that are subject to voluntary measures, as appropriate; Develops intervention plans and intervenes in all cases in accordance with the framework for the child's life project and in alignment with Cree healing practices and paradigms; Intervenes in crisis situations; Provides on-going practical and psycho-social supports to encourage families address their issues and aims at finding solutions to their problems by undertaking activities that require individual effort; to collaborate with internal and community resources which further the same goals; Assesses and assists in selecting the best alternative when a child cannot be maintained in his/her natural milieu and to prepare them for the placement of children outside the family unit, and/or permanency planning where required; Reintegrates or keeps the child in the family environment or indicate the best plan for the child; connect with the child's family circle and home community; Includes and applies reference frameworks, practice guides, programs and protocols; Establishes and maintains collaborative relationships with colleagues and partners, both internally and externally (e.g. CMC; schools; Foster Care resources; Youth Healing Services; Eeyou Eenou Police Force; community programs; etc.); Participates in meetings and orientation tables, depending on the request; Testifies in Youth Court as person authorized by the Director of Youth Protection; Records notes in cases according to legal requirements and the administrative requirements of CBHSSJB; Observes confidentiality at all times and for all files.   • SUMMARY OF THE POSITION
    Person who implements, updates, monitors, analyses and assesses clinical activities and intervention plans on behalf of the Director of Youth Protection and in collaboration with Youth Healing Services.
    SPECIFIC FUNCTIONS
    The holder of this position provides specific clinical, professional and case management services, support and advice to the youth protection community workers. This role involves support to Youth Protection community workers by ensuring: His/her own and the team's responsibilities according to the Youth Protection Service's programs, policies, procedures and program manuals, and including the codes of ethics, agreements of confidentiality, and the policies regarding access to client files; The staff's implementation of the policies, standards, and quality assurance program of the professional practice and clinical activities; The role is a staff/functional one within the principle that the primary responsibility and decision making case management and services mandates as well as direct linkage with their Directors; He/she also assists the DYP/PD in the development and implementation of policies and procedures. Specific social assistance team leader role (HRO): To provide support to the Community Workers in work related matters; To oversee the worker's workload, schedule, timesheets are submitted; To oversee the service is provided 24 hours 7 days a week for the public; To conduct one on one, monthly meetings and training to new employees; To oversee the reports being retained/not retained or referred to other services; To identify issues with service delivery and problem solving on how to ameliorate services; To insure priority cases are processed in the shortest delay and directed to the local team leaders.     Education: Depending on the given sphere of activities, must have the required university degree in social sciences, such as criminology, social work, sexology or psychology; A human relations officer who has the required university degree in social work and is a member of the Ordre professionnel des travailleurs sociaux du Québec may use the title of professional social worker.   • SUMMARY OF THE POSITION
    Person who works directly with service users to ensure their education and rehabilitation in an institutional or external setting, in accordance with the intervention programs established in collaboration with the existing team of professionals, to foster the rehabilitation of individuals and/or their reintegration into society.
    She/he applies education techniques involving activities of daily living in organizing, coordinating and conducting programmed activities aimed at helping these individuals learn and acquire appropriate attitudes and behaviors.
    She/he observes and analyses the behavior of the service users, participates in assessing their needs and abilities and records their progress by writing up the appropriate reports. She/he is responsible for programming her/his own activities.
    SPECIFIC FUNCTIONS:
    Working with clients who may be physically and/or intellectually disabled.
    Assisting clients in achieving success according to a care plan which will be developed in collaboration with a multi-disciplinary team.
    REQUIREMENTS
    Education: Diploma of college studies in specialize education technique or delinquency intervention technique from a school recognized by the Ministère de l'Éducation, du Loisir et du Sport; This class also includes those who, after acquiring relevant experience, have obtained a certificate of college studies in special care counselling or institutional rehabilitation.
