    Your Opportunity: The MAiD Coordinator will have responsibility for coordinating and facilitating MAiD services for patients living in Hamilton, who have requested MAiD support.
    About Medical Assistance in Dying (MAiD): MAiD is supported through the following changes: “In February 2015, the Supreme Court of Canada ruled in Carter v. Canada that parts of the Criminal Code would need to change to satisfy the Canadian Charter of Rights and Freedoms. The parts that prohibited medical assistance in dying would no longer be valid. The Supreme Court gave the government until June 6, 2016, to create a new law. In June 2016, Parliament passed federal legislation that allows eligible adults to request medical assistance in dying”.     Duties and Responsibilities: Answering phone calls, providing support to patients and caregivers who may have questions or wish to self-refer. Accept referrals, triage patients and liaise with team assessors/providers. Maintain a regular caseload, estimated 12-15 open cases at any given time. Provide supportive health teaching for patients and caregivers relating to MAiD eligibility criteria and processes. Coordinate, attend and participate in patient assessments with team a team including physicians and/or Nurse practitioners. Coordinate and assist with all aspects of provision (i.e. organize IV starts, drawing up of meds prn, preparing patient and family etc.) Support patients with resources should they wish to donate their organs. Maintain accurate documentation (SOAP notes, MOH forms, outcome tracking etc.), Foster and maintain collaborative relationships with Primary Care/referring MRP. Liaise with community partners (e.g. HCCSS, visiting nurses) as needed. Provide back-up coordination coverage for team members as needed. Participate in quality and performance improvement activities Participate in community tables and planning committees Participate in sustainability planning for the team, considering innovative options to ensure long-term viability of the team.
    Education and Experience: Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario with experience and expertise in intravenous starts. Or Registered Social Worker with experience supporting patients through MAiD and strong knowledge of a palliative approach to care. Experience providing a empathetic approach to care and supporting both patients and their caregivers. 3-5 years of clinical experience preferred
    Specialized skills and knowledge: Strong understanding of all MAiD legislation and processes Strong knowledge of relevant legislation including but not limited to Freedom of information and Protection of Privacy Act, Regulated Health Provisions Act. Exceptional and compassionate interpersonal and communication skills
    Location(s) Office location may vary across Hamilton FHT hub sites. Travel is a requirement of the role.

    Your Opportunity: As a Primary Care Clinical Pharmacist, you will be part of the team in the fast-paced practices of family physicians. You will work collaboratively with other health professionals to provide comprehensive, accessible primary health care, honouring patient choice in how best care will be delivered (in-person, telephone, video). Pharmacists in the Hamilton Family Health Team (HFHT) aim to improve drug therapy outcomes using a collaborative care model that integrates pharmacists into the primary care team.
    As a pharmacist working in this Family Health Team, you will: Integrate into various family medicine group practices by collaborating with physicians, other health care professionals and patients to promote optimal, patient centered care Provide individual patient assessments to identify, prevent, and resolve drug-related problems Provide pharmaceutical consultation, pharmacotherapy monitoring, education/teaching and drug information Support health promotion, disease prevention and chronic disease management Develop the practice based on the needs of the FHT
    To be eligible, you must: Have a Bachelor of Science degree in pharmacy or PharmD from a recognized university Be registered and in good standing in with the Ontario College of Pharmacists (Part A) Have completed a community or hospital residency or have equivalent clinical experience in either setting* Be able to work in an interdisciplinary team environment Be able to prioritize and be flexible in a very active work environment Demonstrate expertise in practicing pharmaceutical care, problem solving and critical thinking

  • The Hamilton Family Health Team is one of the largest Ontario Health Teams in the province.
    The Clinical Coach is responsible for working with participating community organizations in alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. The Clinical Coach will drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the Community ("The Delivery Framework") and the Ontario Palliative Care Competency Framework.
    The Clinical Coach will: Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies. Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results. Consult on clinical care when the complexity of the patients’ needs is beyond the existing competencies of providers (in the community organizations). Provide support to Indigenous communities and Indigenous organizations for their implementation efforts, where Indigenous communities wish to be involved, in collaboration with regional partners. Co-create the regional Delivery Framework implementation plan with Ontario Health, including required measurement and reporting. Attend all regional planning meetings and contribute to the regional palliative care implementation plan, especially plans for education and practice change. Provide feedback on key learnings related to coach role, along with providing input and reviewing regional progress reports as required. Foster regional collaboration and contribute to broader system integration. Develop relationships with key partners to establish trust, credibility, and respect. Support Ontario Health Team planning and/or implementation of palliative models of care (where there is readiness). Ensure alignment of community implementation with the guidance of the Ontario Palliative Care Network. Critically evaluate the quality of palliative care in participating community organizations. Lead engagement and community-building activities with participating organizations. Build competency amongst front-line community providers in the delivery of primary-level palliative care. Offer consultation as a palliative care expert to service providers in person, onsite, by telephone, by videoconference or through email. Provide support to Indigenous communities and organizations for their implementation efforts, as requested by those organizations and, where appropriate, in collaboration with regional partners Guide the creation of a quality improvement plan to address gaps. Provide limited and occasional direct patient care.
    Here is what you will need to be successful:
    Post secondary degree in a registered health care discipline (e.g., RN, RSW, NP) with substantive experience and training in palliative care (e.g., CAPCE). A graduate degree is an asset (e.g., MScN, MSW). Minimum five (5) years’ experience in practice in palliative care is required. Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice is required. Experience supervising clinical staff is preferred. Experience working with large-scale quality improvement initiatives. Additional training in gerontology and project management experience are assets. Strong clinical coaching and debriefing skills, with both individuals and teams. Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team and community service providers. Research and/or program evaluation experience with knowledge of evidence-based processes, including appraisal of research, application of findings and collaboration on research. Experience working with a range of roles, disciplines, and levels of staff in different health care settings. Experience working across health care settings. Fluency in French and/or another language is an asset. Ability to work well under pressure and use good judgment in assessing difficult situations. Ability to work independently and as part of a team. Experience designing education to effectively address palliative care training needs of interprofessional learners. Excellent communication skills and experience with culturally responsive teaching. A demonstrated focus on person-centred care. Support the advancement of equity, inclusion, and diversity across Ontario's health care system. Commitment to engage with First Nations, Indigenous, Métis and urban Indigenous people as they self- determine their own approach to palliative care. Proficiency in educational technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel.
    Location(s) The home base for this position will be at Hamilton Family Health Team, 123 James Street North, Hamilton, Ontario L8R 2K8. Access to reliable and efficient mode(s) of transportation to enable travel between community organizations is required.
